The State of Connecticut’s Department of Social Services and Department of Developmental Services manage the home and community-based services which provide assessment, coordination and payment for services for people who have physical or developmental disabilities and who need supportive services to live independently in the community. The programs can pay for non-medical services such as adult day care, chore assistance, home-delivered meals, companion, homemaker and personal emergency response systems. These services may have income and asset restrictions.

HOW TO APPLY:
People with mental disabilities contact DDS Central Intake for services. Other adults with disabilities can fill out an application or apply at DSS field offices, see the link above under “Documents/Forms” for more details.
