Patient Access to Medical Health Information in Connecticut

By law, a health care provider, a hospital and other health care facilities must provide a copy of a patient’s health record upon the written request of the patient, his/her attorney, or authorized representative. Pursuant to Connecticut General Statutes and Regulations of Connecticut State Agencies. Upon a written request of a patient, the patient’s attorney or authorized representative, or pursuant to a written authorization, shall furnish to the person making such a request a copy of the patient’s health record, including but not limited to, bills, x-rays and copies of laboratory reports.

If a patient is having trouble locating their records the Department of Public Health suggests that they contact any associated physician’s office, the hospital affiliated with the practice, or the local health director’s office for possible information about the whereabouts of their records. Connecticut’s Health Information Exchange, Connie a statewide advocacy and Health Information Exchange (HIE), in collaboration with the Connecticut Office of Health Strategy, facilitates the secure and confidential exchange of health information among healthcare organizations such as doctors’ offices, hospitals, labs, and radiology centers, ensuring seamless coordination of care. The Exchange is working to implement health information technology and health information exchange services to make improvements to patient care, by providing secure and authorized access to patient health information and improve accessibility of patient medical information.
